Get Ready for Digital Pills: FDA Clears Self-Tracking Drug

The US Food and Drug Administration (FDA) has approved Abilify MyCite (aripiprazole tablets), which contains an ingestible sensor that records whether or not the medication was taken.

Otsuka said that the pairing of the sensor with the antipsychotic Ability was created to allow patients with schizophrenia, bipolar I disorder or major depressive disorder to better track their daily medication intake and share that with their care team - which they can control through the linked app. From that point, with a patient's consent, the data can be accessed by the patient's doctors or caregivers via a web portal.

Dr Mitchell Mathis, director of the division of psychiatry products at the FDA, said: "Being able to track ingestion of medications prescribed for mental illness may be useful for some patients".

Smart pills are a thing and they've come one step closer to entering the mainstream after the FDA approved a pill packed with a digital sensor for the first time in the US. It also should not be used to track drug ingestion in "real-time" or during an emergency. For this digital pill combination, it partnered with Japanese drug maker Otsuka Pharmaceutical, which makes Abilify.

The new digital pill works by relaying a signal from a tiny sensor embedded in the pill itself (which is activated by the liquid in the human stomach) to a wearable patch on the patient.

US regulators have approved the first drug with a sensor that alerts doctors when the medication has been taken, offering a new way of monitoring patients but also raising privacy concerns. And because elderly people with dementia-related psychosis are at increased risk of death when taking antipsychotic drugs, the drug is not approved in this population, either.

Abilify Mycite's prescribing information notes the ability of the product to improve patient compliance with their treatment regimen has not been shown.

The problem is so pervasive that experts estimate that such behavior winds up costing about $100 billion per year because paitients end up getting sicker and then need more treatment or require hospitalization, according to The New York Times (paywall). A person who suffers from this mental disorder appears to be out of touch with reality.

In the same article, Eric Topol, MD, director of Scripps Translational Science Institute, predicted that payers might eventually offer financial incentives to use digital bills, but anxious about the ethical issues that could present themselves if the if the technology was "so much incentivized that it's nearly is like coercion".

"With this FDA approval, Otsuka can help enable individuals with serious mental illness to engage with their care team about their treatment plan in a new way", he added.
